Thank you for signing up for the MBA All Sports Night benefitting the Leukemia & Lymphoma Society. Please complete this form with your child’s name, grade, age, and emergency contact information. Please also print the attached waiver and have your child bring it to the event.

Just to remind you, please park in the tennis court parking lot near Montgomery Bell Avenue and proceed to the entrance.

Check-in tables are at the SW entrance to the Burkholder Wellness Center, overlooking the Andrews Lacrosse field. Drop-off begins at 6:00 pm, and pick-up begins at 8:30. If someone other than a parent or guardian will pick up your child, please let them know at check-in and have the designated person bring their driver’s license.
